The Clinical DocumentationSpecialist-Clinical is responsible for improving overall quality andcompleteness of clinical documentation in the health record ensuring compliancewith the organizations coding procedures and standards. ClinicalDocumentation Specialist facilitates documentation opportunities for clinicaldocumentation to accurately reflect patient severity of illness and risk ofmortality through extensive interaction with physicians, case management,nursing staff, other patient caregivers and HIM coding staff while striving toeliminate gaps, assisting providers with accurate and complaint documentationresulting in appropriate healthcare reimbursement, and clinical outcomes.
Ensuringthe accuracy and completeness of clinical information used for measuring andreporting physician and hospital outcomes. Reports concurrent chart review andquery outcomes to hospital departments and committees at designated intervals. Specialistsutilize monitoring tools to track the progress of the CDI program; maintainingaccurate records of review activities to comply with departmental andregulatory agency guidelines. Specialists understand and comply with policiesand procedures related to confidentiality of medical records; identifyingopportunities for intradepartmental and interdepartmental operational improvements. Specialists participate in programrelated meetings, physician and staff education, staff development,departmental activities and in-service opportunities.
Strong broad-based clinicalknowledge and understanding of pathophysiology of disease processes andclinical documentation. Excellentwritten and verbal communication skills, critical thinking skills, listeningskills and interpersonal skills to build effective partnering relationshipswith physicians, care management team, nursing staff, coding staff and otherhospital staff. Able to present ideas and concepts effectively to physicians,management and employees. Ability to work independently in a time oriented environment.Ability to analyze problems and issues from a variety of perspectives andunderstand the legal, reimbursement and impact.
- RN License valid in the state of Utah
- Five years of experience in an Acute Care setting.
- Knowledge of care delivery documentation systems and related medical records documents.
- Prior experience in clinical documentation improvement programs preferred.
- Able to communicate effectively in English both verbal and written.
- CCDS certification or CDIP certification preferred
- Must attain CCDS or CDIP within one year of employment
- RNs with case management experience preferred
- RNs with utilization review experience preferred
