The Case Manager manages the collaborative interdisciplinary team process which assesses, plans, implements, monitors, and evaluates the options and services required to meet an individual's health care needs. The Case Manager must be able to demonstrate knowledge and skills to provide care appropriate to the age of the patient's served across the continuum. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret appropriate information needed to identify each patient’s requirements relative to his or her age specific needs. Must be able to respect diversity of each individual’s values, culture, spiritual beliefs, and other socio-economic issues in addressing the needs of patients, visitors, and co-workers. The Case Manager must be familiar with the admission, discharge, and transfer criteria for each Medical Center unit. Will utilize communication and available resources to promote quality, cost effective outcomes. Must effectively interact with patients and health team members while maintaining established professional standards. Other duties as assigned.
- Must be a high school graduate.
- Must be skilled in assessing, planning, and implementing patient care.
- Behavioral health background preferred.
- Ability to read and communicate effectively in English. Additional languages helpful.
- Strong written and verbal skills.
- Basic computer knowledge with typing and word processing skills, excellent verbal and written communication skills, and ability to perform independently.
